Legend of Zelda: Parallel Worlds
Some crazy guys on the internet have just recently finished (see: released) one of the largest hacks I've seen of any ROM in a long time.
They went into the original SNES Legend of Zelda: A Link to the Past, rewrote the story, changed the map, updated and changed the art, and added new and different NPC's.
The game, entitled "Parallel Worlds", can be applied to an existing ROM file of Link to the Past with a patching program like LunarIPS. Then you boot directly into the modified world of SNES adventuring goodness.
I highly recommend checking this out, these guys did a great job.
